This in turn causes a reaction of the immune system which initiates the migration of neutrophils and lymphocytes to the site of injury and creates pus and thus acne breakouts. This process may begin during puberty and continue into adulthood. During puberty our body starts to produce hormones called androgens. Androgens cause the sebaceous glands to enlarge (hormonal acne may result) and become overactive. This process is part of normal development. The sebaceous glands become over stimulated by androgens and release a large amount of sebum. The sebum outflow injures some of our cells and the inflammatory process that ensues leads to acne. Extra Sebum Extra sebum contributes to acne as well. After the sebaceous gland is stimulated by androgens, it produces extra sebum. The sebum moves up the follicle and mixes with dead skin cells that have been shed from the lining of the follicle. This increases the chances of clogged pores, which can result in acne. Follicle Fallout Follicle fallout is another possible cause of acne. When dead cells within the follicle shed more rapidly, they mix with the extra sebum, stopping the renewal process. The renewal process is important to keep your skin functioning properly. Bacteria A clogged pore becomes the perfect breeding ground for bacteria. The bacteria tht lives in symbiosis with us in the sebum glands start to reproduce uncontrolled and create chemicals that cause our immune system to fire a rescue army of its own chemicals and immune cells, a reaction known as inflammation. This reaction is what causes pimples to become red, painful and swollen. Inflammation Inflammation is a natural process by which the body tries to rid itself of a foreign object or infection. However, it can also do damage. Comedones are considered to be the only form of non-inflammatory acne. Whiteheads are closed comedos because they usually appear as small white bumps. An open comedo is called a blackhead pimple because it has a dark appearance which is due to a follicle being pushed above the skin’s surface and getting mixed with melanin pigments or becoming oxidized. Papules, considered the mildest form of inflammatory acne, appear as small solid bumps that can sometimes be tender. Pustules are small round lesions with visible pus. They are caused by the chemical irritation to fatty free acids released by bacteria that has gone uncontrolled. Nodules are large, extremely painful pus-filled lesions lodged deep within the skin. Nodules develop when the inside of a comedo spills into the surrounding skin, causing the immune system to produce pus. Cysts are nodules that have become severely inflamed lesions. They can last for weeks or even months. The lesion usually hardens, becoming a deep cyst. Nodules and cysts can result in deep acne scarring.
